The ConneticsCare Process: Three Simple Steps

Step 1: Your physician determines that EVOCLIN is the appropriate therapy for you.

Step 2: Your physician gives you the EVOCLIN prescription; the nurse or office staff provides the ConneticsCare helpline number and rebate coupon.

Step 3: If unsure of coverage, you can call the ConneticsCare helpline before going to the pharmacy. A Reimbursement Specialist addresses coverage issues. You may also call the pharmacy to check insurance coverage.
